All Physical Forms Must Go Through Rank One

  • As of March 2022, GCPS requires athletes to use the Rank One portal to submit physical forms and other documents.

    ATTN: Sports Broncos and Their Parents

    Please follow these instructions to establish a Rank One account and submit all necessary paperwork for sports tryouts and membership. 

    * Before proceeding, review the Rank One Parent Tutorial at this link.

    Then follow these steps:

      1. First, click here to create a Rank One parent account.

      2. Second, fill out the patient information and health history portions of the online GHSA Physical Form and print it. (See links to the form below.)

      3. Bring the partially completed printed form to your physical appointment for the doctor to complete the rest. 

      4. Once the form has been completed by a doctor, scan and upload it to your Rank One account.
