Health Management Plans
The parent/guardian will need to complete a health management plan for students with medical conditions that may require assistance and/or monitoring during school hours (i.e. asthma, severe allergies, diabetes, etc.). Please contact Mrs. Pickens or Mrs. Rhodes for all health management plans.
Medications
Administration of prescription and over-the-counter medications during school hours is discouraged. Medications prescribed three times daily often can be given before school, after school, and at bedtime. However, if administration of the medication is required, the parent must complete the Administration of Medication Request Form. Students may not have prescription and/or over -the-counter medications in their possession. ALL medications must be kept in the clinic.
IMPORTANT:
- Prescription medication must be in the original prescription container.
- Over-the-counter medication must be in the original container.
- All medications not picked by the end of the school year will be discarded.
Dismissal from Clinic
Students who become ill or injured during the school day should request a pass from their teacher to go to the clinic. Clinic personnel will contact a parent or guardian as situations require. All students who require checkout due to illness or injury must check out through the clinic
