Dear Parents,
The clinic is available to assist with contacting the parents of sick children, dispensing daily medications, treating minor injuries by providing minor first aid, and offering emergency care until medical professionals arrive. To help students with their individual medical needs, please take a moment to request and fill out the appropriate Health Management Plan and Administration of Medication forms. The Health Management Plan will need to be filled out if your child has been diagnosed by a physician. It means so much to me to have well communicated health plans so that I may address student health needs accordingly. Some common medical conditions are listed below: (if these do not apply, please contact the clinic to discuss your need)
- Allergies (food, seasonal, chemical, etc.)
- Asthma
- Bleeding Disorder
- Cancer
- Cardiovascular Issues
- Cystic Fibrosis
- Inflammatory Bowel Disease
- Migraines
- Seizures
- Sickle Cell
- Vagal Nerve Stimulator
- V-P Shunt
Parents should check with their physician regarding the need for over the counter and/or prescription medications to be administered during school hours. Medications prescribed three times daily often can be given before school, after school, and at bedtime. If your child requires long term medication, several weeks to indefinite in length, our clinic staff will dispense medication only after your doctor has written specific instructions that include exact dosage and time to be given. All medications must be in the ORIGINAL CHILD PROOF CONTAINER with the prescription label attached to it. Medications stored in envelopes, baggies, etc., will not be administered. All medications and cough drops must be brought to the clinic by a parent/guardian and not transported by the student for safety precautions. The safety of your child as well as other children is the primary consideration behind this rule.
