Information
Creekland Middle School cares about the health and welfare of ALL students. A clinic worker is available during school hours to assist with your child’s illness or injury while at school.
It is imperative to update telephone numbers and emergency contacts with the school so that, in the event of an emergency involving your child, we can reach you quickly.
Medical Forms
Please provide the school clinic with all your student’s past and current medical information relevant to his/her well-being to care appropriately for him/her during school hours by completing the appropriate medical form. If you need a clinic form, you can obtain it from the CMS clinic.

Medications
All medications, both prescription and over the counter, must be accompanied by a School Administration of Medication Request form (available from CMS clinic) and brought directly to the clinic. Students are not allowed to carry any medications on them, including asthma inhalers, during school hours without a physician’s signature on the corresponding health management plan (or a letter written by your doctor authorizing the use of medication while in the classroom). All medications must be kept in the original labeled container. Medications stored in envelopes, baggies, etc., are not to be administered or allowed at the school. Medications brought to school during the school year must be picked up at the end of the year.
All students entering the 7th grade must have a valid Georgia Form 3231 meeting the new requirements. You can read about it following the link below:
