Scoliosis Screening - March 25th
Dear Parents and Guardians,
We are pleased to announce that Snellville Middle School will host our annual scoliosis screening for 6th and 8th graders on Tuesday, March 25th. The screening is done yearly in the middle school grades as required by State Law, Department of Community Health, and State Department of Education guidelines. Acknowledgment forms will be sent home between January 4th – 8th. You may opt out of screening for your student by signing and returning this form. Please follow up with your student to verify receipt of the form and return to school.

**If your child has been screened by a healthcare provider or is under the care of a physician for scoliosis, please submit the completed 4400 form.What is Scoliosis?
Scoliosis is spine curvature that affects 1-2% of adolescents. Early detection and treatment can prevent the curvature from worsening and causing lasting complications.
Why are Screenings Important?
The screenings, required by state law, help us identify students with scoliosis so they can receive the necessary evaluation and treatment.
Screening Details:
- Date: Tuesday, March 25th
- Grades: 6th and 8th
- Location: Snellville Middle School
- Procedure: A simple observation test conducted by trained healthcare professionals. Students will stand and bend forward while their back is examined.
- Privacy: Boys and girls will be screened separately in private settings.
Preparation:
- Girls should wear a sports bra or swimsuit top for accurate screening.
- Please inform the school nurse if your child has health concerns that may affect the screening.
What Happens After the Screening?
If the Screener identifies a potential curvature, you will be contacted by the Gwinnett County Department of Public Health. They will provide information about further evaluation and next steps.

Registration is Tuesdays & Thursdays from 9:00 a.m. to 2:00 p.m. starting July 6th and will be open from 8:00 to 4:00 starting July 24th, 2023.

These additional documents will help us place your child correctly.
- Withdrawal forms from the previous school.
- Transcript or report card from the previous school.
- Discipline record from the previous school or a document from the previous school stating the following: “Student has no discipline record.”
- Individual Education Plan (IEP) - Required if your child receives services through special education.
- ESOL Students (Speakers of Other Languages) – International Center for Documentation and ESOL Documentation. If you don’t have this documentation, please call The International Newcomer Center at 678-985-5200 to set up an appointment for your child to be tested.
- Gifted Students: Gifted Eligibility Report. Students coming from out of state must be tested.
- If divorced or separated, legal documentation showing a registered parent as the primary caregiver.
- Temporary Guardianship: A Letter of Guardianship is required for registration if the enrolling adult is not the birth parent. Contact Gwinnett County Probate Court (770- 822-8350) to obtain guardianship information. The court is located at 75 Langley Dr. in Lawrenceville.
- Registering parent’s Photo ID.
