Clinic Forms

Parents/ guardians must fill out the appropriate clinic form(s) and bring the medication to school with the correct forms completed. Please email our school clinic worker to obtain a copy of the forms that you will need to bring to school.

Any student that needs medication to be administered at school should have the following form completed:

1. Medical Administration Form

Students with Asthma should have the following forms completed:

1. Allergy Plan
2. Medical Administration Form

Students with Allergies should have the following forms completed:

1. Allergy Plan
2. Hypodermic Injection Permit Form
3. Medical Administration Form
